Entrance Requirements
The entrance requirements are listed below:
Note:
Prospective students have to apply for admission. The requirements listed below must be submitted online as part of application requirements unless otherwise indicated. An application fee of Php 400 is charged per applicant. Proof of its payment must also be submitted online.
Preschool
The Children's House
- A completed application form accessed through the MIS parent portal
- 1 ID photo (2 X 2 inches)
- An original Birth Certificate from the Philippine Statistics Authority (PSA)
Kinder
The Children's House
- A completed application form accessed through the MIS parent portal
- 1 ID photo (2 X 2 inches)
- An original Birth Certificate from the Philippine Statistics Authority (PSA)
To be submitted upon enrollment:
- If the student applicant has attended a previous school, a copy of the school’s Report Card (DepEd Form 137) must accompany the application.
Special
Education
The Children's House
- A completed application form accessed through the MIS parent portal
- 1 ID photo (2 X 2 inches)
- An original Birth Certificate from the Philippine Statistics Authority (PSA)
- Diagnostic Report
- If the child has had previous schooling, a Narrative Report.
To be submitted upon enrollment:
- If the applicant has attended a previous school, the original school report card (DepEd Form 137)
Elementary,
Junior High School,
Senior High School
- A completed application form accessed through the MIS parent portal
- 1 ID photo (2 X 2 inches)
- An original Birth Certificate from the Philippine Statistics Authority (PSA)
- Certificate of Good Moral Character

Additional Requirements for Remote Learning
For any Level:
Students admitted into remote learning must submit one of the following documents:
- For students with health conditions: a medical certificate
- For students with physical proximity problems: a residence certificate or proof of residence
- For students with mobility issues: proof of athletic training or letter from parent attesting to residential mobility
Additional Requirements for Foreign Students
For any Level:
- Alien Certificate of Registration (ACR)
- Photocopy of applicant’s passport
For students admitted into blended learning:
- Vaccination Card
- Parent Consent and Waiver
For blended learning in MCH, requirements include:
- Copy of vaccination card of adult companion (required for preschool, optional for kinder)
- Copy of vaccination card for blended learners (if vaccinated)
- Signed parent consent and co-responsibility undertaking
